BILIARY MICROBIOTA AND BILE DUCT DISEASES

Abstract

Traditionally, the biliary tract has been considered to be normally sterile, and the presence of microorganisms in bile is a marker of a pathological process. This assumption was confirmed by failure in allocation of bacterial strains from the normal bile duct. The paper provides rationale for a phenomenon of the normal biliary microbiota as a separate functional layer which protects a biliary tract from colonization by exogenous microorganisms. We revealed the potential of metagenomic data for prevention of infectious diseases, post-operative complications of reconstructive interventions including bile duct stenting and implantation the tissue-engineered structures exposed to the risks of colonization with pathogenic / exogenous microorganisms. The methods based on preserving homeostasis of normal biliary microbiota ecosystem can be used for prevention of hepatobiliary diseases and treatment of biliary tract inflammatory diseases.

QUALITY EVALUATION OF HEALTHCARE SERVICES IN SCHOOLS

Abstract

Background: School healthcare service standards and quality are the crucial factors for a development of a healthy lifestyle health of younger generation of the country. The goal of the study is an evaluation of the health care quality in schools.

Methods: The paper discusses the evaluation of the quality of health care which is provided for school children in different parts of Russia. Assessment of the school health services is performed using the unified WHO recommendations in 21 schools.

Results: The study revealed the similar pattern of problems concerning health services in schools located in different regions of the country, specifically: the absence of modern federal rules; shortage of medical professionals and their low sallary; insufficient medical equipment; inadequate list of medical services.

Conclusions: This study gives the evidence for the necessity to develop a general strategy of measures to be implemented to solve the problems. The measures should include the regular trainings of medical professionals, determination of the list of basic services to be provided in schools, improvement of the medical equipment.

CHALLENGES OF MOLECULAR-BASED DIAGNOSIS DEVELOPMENTAL DYSPLASIA OF THE HIP IN CHILDHOOD

Abstract

Developmental dysplasia of the hip (DDH) is a vital and most frequently diagnosed disorder in pediatricrequiring developement and implementation of new diagnostic methods because of failure in detection of early dysplastic changes. Limitation in applying the diagnostic instrumental methods in infants and lack of uniform standards of clinical practice and management results in the increase in treatment cost and duration of rehabilitation. The purpose of this study was the search for new sensitive methods of diagnostics and treatment of DDH and profound study of pathogenic mechanisms. Despite the great attention of international pediatric specialists to the problem of DDH in children, the methods for early diagnostics, prevention of severe forms and complications are not still developed. The current study identifies the molecular mechanisms of DDH pathology and analyses the opportunities of instrumental and non-instrumental methods for its diagnosis. The authors presented the promising biomarkers and cytokines for the diagnosis and prediction of DDH.

THE JUSTIFICATION FOR THE USE OF THE THERAPEUTIC FACTORS OF THE INTERNAL ENVIRONMENT OF MODERN DEVICES MADE OF NATURAL SYLVINITE IN DENTISTRY

Abstract

Background: One of the methods of comprehensive therapeutic effects on the human body is sylvinite therapy which has not been used in the treatment of inflammatory periodontal diseases at young age. Aims: Hygienic and clinical evaluation of the possible application of therapeutic factors of sylvinite devices in dentistry.

Materials and methods: We conducted hygienic study of the basic physical factors of the internal environment of modern sylvinite structures (radiation background, aeroionization and aerosol environment, climate). The percentage of sylvine and halite in the sylvinite minerals was determined using color image analysis of the salt screen. The microbiological studies of the impact of the mineral sylvinite on the growth parameters of bacterial culture were performed. We examined 79 patients aged 20−25 years diagnosed with chronic generalized catarrhal gingivitis, 36 persons underwent treatment course in the salt structure. The evaluation of the dental status and the periodontal status was conducted using hygienic and periodontal indices. The duration of study was 4 years.

Results: Аll participants who entered the study completed it. In the experimental sylvinite room formed food-grade, biodegradable internal environment (background radiation ― 0,18±0,0027 µsv/h; LOA ― 802,33±62,69 ion/cm3; RO ― 509,33±37,17 ion/cm3; stable microclimate). We detected evidence of an inhibitory effect of the combination on Staphylococcus aureus. The use of a comprehensive dental program has improved the condition of periodontal tissues and careeradvantage tooth enamel (PMA and SBI fell of 80.8% and 75.5%; the reduction of the CPU ― 67.7%; fuel and energy resources increased by 37%). Conclusions: The inclusion of sylvinite therapy in the complex treatment of patients with chronic generalized catarrhal gingivitis had a marked positive impact on the state of periodontal tissues and local immunity of the oral cavity.
