Цитокины крови при псориазе: связь с клиническими индексами тяжести заболевания и наличием псориатического артрита

Аннотация
Обоснование. Патогенез псориаза связан с дисрегуляцией иммунной системы и воспалением, что проявляется существенными сдвигами цитокинового профиля пациентов. Данный факт может быть использован для мониторинга течения заболевания и его лечения, однако результаты настоящих исследований недостаточны, иногда противоречивы и требуют более углубленного анализа.
Цель исследования — выявление клинико-лабораторных соответствий между цитокиновыми профилями, клиническими индексами тяжести псориаза и наличием псориатического артрита (ПсА).
Методы. Для оценки степени тяжести псориаза использовали стандартизированные клинические индексы — PASI, BSA, sPGA. Степень тяжести псориаза оценивали как среднюю при 10 ≤ PASI < 20, sPGA — 2–3, как тяжелую — при PASI ≥ 20, sPGA — 4–5. Определение уровня цитокинов в плазме крови больных проводилось методом мультиплексного иммунологического анализа с использованием технологии xMAP. Статистический анализ и визуализация полученных данных проведены с использованием RStudio for MacOS и языка программирования R.
Результаты. В исследование включено 113 больных обыкновенным псориазом средней и тяжелой степени тяжести. В соответствии со значениями PASI среднетяжелая степень псориаза констатирована у 55 (48,7%) пациентов и тяжелая — у 58 (51,3%) пациентов. ПсА был диагностирован у 41 (36,3%) пациента. При оценке уровней цитокинов плазмы крови в зависимости от степени тяжести заболевания выявлены различия в уровнях ИЛ-12, ИЛ-20 и ИЛ-22. Показаны статистически значимые различия в уровне ИЛ-6 между группами пациентов с и без ПсА. Выявлены две независимые цитокиновые сети, представленные кластером цитокинов ИЛ1β–ФНОα–ИЛ-17А, ИЛ-22 и ИЛ-20, связанным с клиническими индексами тяжести псориаза, и кластером ИЛ-21, ИЛ-23, ИЛ-25, ИЛ-17F, ИЛ-31, ИЛ-33, ИЛ-4 и ИЛ-10, не связанным со степенью тяжести псориаза.
Заключение. Результаты проведенного исследования впервые описывают структуру цитокиновых сетей, связанных с формированием системного воспаления при псориазе, а также характеризуют основные эффекторные цитокины, определяющие выраженность воспалительной реакции в коже и развитие ПсА.
