The rate of endovascular versus microsurgical treatment for CA of different locations in the era of rapidly emerging endovascular medicine is unknown. </italic><bold><italic>Aim</italic></bold><italic>: To study the frequency of microsurgical or endovascular techniques for the treatment of CA of different locations. </italic><bold><italic>Methods</italic></bold><italic>: Methods of treatment and localization of CA were studied in meta-analysis of clinical series published from 2003 to 2014. Case-control studies, studies with externally balanced number of patients in the groups, and the series in which a large number of patients were treated out the study were excluded. </italic><bold><italic>Results</italic></bold><italic>: 1 international, 2 American, 2 Japanese and 3 Russian clinical series (n=5254 CA) were included in the meta-analysis. The pooled rate of microsurgical treatment used for the CA of the internal carotid artery was 65% (95% CI 55−75), the anterior cerebral artery </italic>―<italic> 65% (95% CI 46−84), the middle cerebral artery </italic>―<italic> 90% (95% CI 82−98), and vertebrobasilar basin </italic>―<italic> 39% (95% CI 41−64). </italic><bold><italic>Conclusions: </italic></bold><italic>In clinical series both methods of CA treatment were available but endovascular closure was used for the majority of vertebrobasilar basin aneurysms, and for more than a third of anterior cerebral artery or internal carotid artery aneurysms. Middle cerebral artery aneurysms, as opposed to CA of other locations, were subjected to microsurgical treatment in the most cases (90%). In some cases CA are not suitable for endovascular closure, or require microvascular reconstructive operations. In competition with less invasive but more expensive option of endovascular treatment, and under the conditions of decreasing volume and experience of open CA surgery, microsurgical techniques should be mastered to a high level which requires centralization of the patients in the specialized centers and microneurosurgical training. </italic></p></abstract><trans-abstract xml:lang="ru"><p><italic>До недавнего времени основным способом выключения церебральных аневризм (ЦА) из кровотока являлся метод микрохирургического клипирования. Частота применения эндоваскулярного либо микрохирургического закрытия ЦА в зависимости от локализации в эпоху активно развивающейся эндоваскулярной нейроинтервенционной медицины неизвестна. </italic><bold><italic>Цель: </italic></bold><italic>изучить частоту применения микрохирургического либо эндоваскулярного метода лечения ЦА в зависимости от локализации. </italic><bold><italic>Методы. </italic></bold><italic>Проведен метаанализ клинических серий лечения разорвавшихся и неразорвавшихся ЦА, опубликованных с 2003 по 2014 г., в которых имелись сведения о методах лечения и локализации ЦА. В анализ не включали исследования типа случай−контроль; исследования, внешне сбалансированные по числу наблюдений в группах, и серии, в которых значительное количество пролеченных пациентов исключено из анализа. </italic><bold><italic>Результаты. </italic></bold><italic>В метаанализ включены 3 отечественные и 5 зарубежных клинических серий, в которых представлены данные о лечении 5254 ЦА различной локализации. Суммарная доля микрохирургического метода в лечении ЦА внутренней сонной артерии составила 65% (95% ДИ 55−75), передней мозговой артерии </italic>―<italic> 65% (95% ДИ 46−84), средней мозговой артерии </italic>―<italic> 90% (95% ДИ 82−98), вертебробазилярного бассейна </italic>―<italic> 39% (95% ДИ 41−64). </italic><bold><italic>Выводы. </italic></bold><italic>В клинических сериях, где доступны оба метода лечения, эндоваскулярный способ закрытия использован для большинства ЦА вертебробазилярного бассейна, более 1/3 ЦА передней мозговой и внутренней сонной артерий. ЦА средней мозговой артерии, в отличии других локализаций, в большинстве случаев (90%) подвергались микрохирургическому лечению. В ряде случаев ЦА технически не подлежат эндоваскулярному закрытию, требуют реконструктивных микрососудистых операций.
