Conventional therapy including systemic corticosteroids with or without other immunosupressants causes numerous adverse effects and becomes inefficient in refractory patients. In this work, the authors showed a modern view on the pathogenesis of pemphigus. This article describes the detailed action mechanism of rituximab, a chimeric monoclonal antibody directed against CD20 antigen of B-cells. The authors conduct the results of meta-analyses of rituximab’s efficiency in pemphigus patients. Moreover, in this article, the authors consider new promising treatment options and potential targets for biological therapy of pemphigus diseases.</italic></p></abstract><trans-abstract xml:lang="ru"><p><italic>Пузырчатка — тяжелое, потенциально летальное заболевание, обусловленное продукцией десмоглеиновых антител и иммуноопосредованной регуляцией Т-клеточных популяций. Традиционное лечение, включающее системные глюкокортикоиды в виде монотерапии или в комбинации с другими иммуносупрессивными препаратами, вызывает ряд серьезных нежелательных эффектов, а также неэффективно при резистентных к системной терапии формах заболевания. В статье представлены современные данные о патогенезе пузырчатки, рассмотрен механизм действия препарата химерных моноклональных антител к </italic><italic>CD</italic><italic>20 антигену В лимфоцитов ритуксимаба. Приведены результаты метаанализов клинических исследований по изучению эффективности ритуксимаба у больных пузырчаткой.
