In 63 pregnant women, hemostatic disorders were not corrected, in 35 patients medical tactic defined clinical situation and the results of laboratory and instrumental studies of the hemostatic system. <bold>Results</bold>: Hereditary thrombophilia detected in 85% of patients with obstetric complications. Carriage of the rare allele of MTHFR-677 2-fold increased risk of adverse pregnancy outcomes; heterozygous carriers of polymorphism PAI-1 — 1,5 times more. The prognostic parameters thrombelastographic (sensitivity 80%, specificity 82%) were higher than in laboratory tests. To determine the most predictive of adverse pregnancy outcome has a density of fibrin clot — the maximum</italic><italic> amplitude. <bold>Conclusions</bold>: In women with obstetric complications associated with disturbances in the hemostatic system, medical diagnostic tactics using thrombelastographic evaluates all stages of coagulation and fibrinolysis, allows you to select a reasonable pathogenetic anticoagulant therapy and reduce the number of adverse pregnancy outcomes.</italic></p></abstract><trans-abstract xml:lang="ru"><p><italic><bold>Цель исследования</bold>: добиться уменьшения числа неблагоприятных исходов беременности у женщин с генетически детерминированными</italic><italic>и гестационными изменениями в системе гемостаза. <bold>Пациенты и методы</bold>: основную группу составили 98 пациенток с акушерскими</italic><italic>осложнениями на фоне изменений показателей скрининговой коагулограммы. У 63 беременных нарушения в системе гемостаза</italic><italic>не корригировались, у 35 пациенток лечебная тактика определялась клинической ситуацией и результатами лабораторно- инструментального исследования системы гемостаза. <bold>Результаты</bold>: наследственные формы тромбофилии выявлены у 85% пациенток</italic><italic> с акушерскими осложнениями. Носительство редкого аллеля гена MTHFR-677 в 2 раза увеличивает риск развития неблагоприятных</italic><italic> исходов беременности; у гетерозиготных носителей полиморфизма PAI-1 риск в 1,5 раза выше. Прогностические возможности</italic><italic> параметров тромбоэластографии (чувствительность 80%, специфичность 82%) оказались выше, чем у лабораторных тестов.</italic><italic>Наибольшей прогностической возможностью при определении неблагоприятного исхода беременности обладает показатель плотности фибринового сгустка — максимальная амплитуда. <bold>Выводы</bold>: у женщин с акушерскими осложнениями, ассоциированными с нарушениями в системе гемостаза, лечебно-диагностическая тактика c использованием тромбоэластографии позволяет оценить все этапы свертывания и фибринолиза, выбрать патогенетически обоснованную антикоагулянтную терапию и уменьшить число неблагоприятных исходов беременности.</italic></p></trans-abstract><kwd-group xml:lang="en"><kwd>obstetric complications</kwd><kwd>hemostasis</kwd><kwd>genetic thrombophilia</kwd><kwd>antithrombotic therapy</kwd><kwd>thromboelastography</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>акушерские осложнения</kwd><kwd>гемостаз</kwd><kwd>генетическая тромбофилия</kwd><kwd>антитромботическая терапия</kwd><kwd>тромбоэластография</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>1.
