The role of stationary and dynamic genomics was analyzed,</italic><italic>especially molecular visualization that allows the earliest possible intervention. The article also includes examples of molecular visualization use</italic><italic> in diagnosis of myocardial dysfunction, disease monitoring, and treatment efficacy assessment. Authors give an analysis of targeted treatment</italic><italic> methods on the example of targeted delivery of medications to the target-organ (myocard). Discuss means of anti-ischemic myocardial protection,</italic><italic> perspectives of metformin use in order to enhance efficacy of myocardial ischemic pre- and postconditioning mechanisms. Presented perspectives of</italic><italic> study of molecular and genetic mechanisms involved in the pathogenesis of HF in diabetic patients, in particular, study of key biological features of</italic><italic> stem cells, cell interactions, stem cell plasticity (in vitro direction of differentiation) and their paracrine function evaluation. Given information about</italic><italic> identification of genes with partly altered expression due to chronic exposure of mesenchymal stem cells to the high concentration of glucose, and</italic><italic> upon decreased ability of mesenchymal stem cells of proangiogenic factors with simultaneous increase of inflammatory markers production (IL8). In</italic><italic> whole this article reviews modern state of HF in diabetic patients development mechanisms study with the use of molecular and genetic technologies,</italic><italic> and of perspectives of development of this area.</italic></p></abstract><trans-abstract xml:lang="ru"><p><italic>В статье освещены особенности развития и течения сердечной недостаточности (СН) при сахарном диабете (СД), влияние диабетической кардиопатии на формирование СН обсужден вклад генетических предикторов развития СД и СН и предикторов ответа на терапию, значение «персонализации» воздействий для оптимизации лечения. Проведен анализ роли стационарной и динамической геномики, в частности молекулярной визуализации для обеспечения максимально раннего вмешательства. Даны примеры ее использования для диагностики патологических состояний миокарда, мониторинга заболеваний, оценки эффективности терапии. Дан анализ методов направленной терапии, на примере направленной доставки лекарственных препаратов в орган-мишень (миокард). Обсуждены технологии защиты миокарда от ишемии, перспективы использования метформина с целью повышения эффективности механизмов ишемического пре- и посткондиционирования миокарда. Представлены перспективы изучения молекулярных и генетических механизмов, вовлеченных в патогенез СН при СД, в частности</italic><italic> изучения ключевых биологических свойств стволовых клеток (СК), клеточных взаимодействий, пластичности СК (направления дифференцировки in vitro) и оценки их паракринной функции. Представлены данные по идентифицикации группы генов, экспрессия которых была специфично изменена при хроническом воздействии на мезенхимные СК (МСК) высоких концентраций глюкозы, и о снижении способности МСК больных СД секретировать проангиогенные факторы с одновременным повышением продукции маркеров воспаления (IL 8).
